The Horn of Ulph
The item is a plate from Vetusta Monumenta (see Bibliography).The engraving shows the horn of Ulph, at York Minster. The inscription on the horn records its history, and its restoration to York Minster in 1675 by Henry Fairfax, who also repaired or embellished it by adding the mounts.
